- To be informed of any matters that concern you including your medical condition and choices of treatment.
- To have access to your personal health information.

The right to express yourself freely.
- To be informed about about how to express concern or make suggestions without fear of anything negative happening to you.

The right to make decisions about your personal or financial life or to choose someone else to act for you.
- To choose whether or not you wish to participate.
